What is the process to apply for a Service Dog?

First, potential clients submit a letter, by mail or on-line request form, outlining the effects of autism/PDD on their child's life and how they feel a dog would help the child in their daily life. Upon receipt of this letter, NSD will assess the information provided and determine if an application will be sent. Children must be between the ages of 2 and 8 and have a diagnosis of autism or other related PDD. If the family is eligible an application will be sent out by e-mail or regular mail. The application includes a number of components:

    a 14 page application form
a letter from each parent showing they agree with and support the decision to get a dog
a number of references from people involved in the child's life

Once the completed application (including references) is received, the client will receive a letter acknowledging receipt of the application. The application will be reviewed and if successful the applicant will go on our interview list.

Next an interview will be set up in the family home at which a trainer will bring a dog and meet your child, and discuss any further questions or concerns. Final acceptance is determined following this interview and families maybe disqualified at any time durng the application process.

Once approved for a service dog families will have the opportunity to attend a general information session. This seminar will give families the opportunity to learn more about NSD and prepare them for the next stages in the process. Families who are not in driving distance of Cambridge will have the information mailed to them.

How long does it take to receive the dog?
Currently it takes 3 years, from time of acceptance, to receive a service dog.

    How is NSD funded?
NSD is a non-profit charitable organization that operates fully on donations from individuals, corporations, service clubs and foundations. Staff, volunteers and clients work in partnership to raise the necessary funding for NSD's programs.

How will our Service Dog be funded?
Applicants are encouraged to help NSD raise the funding for their dog by putting on fund-raising events or providing contact information of organizations or individuals in their communities that might be willing to provide the funding for the dog. Fund-raising activities are voluntary. Many families who have opted to help with fund-raising have experienced an overwhelming sense of community and have helped to educate the public about NSD and autism.

The actual direct cost of the Service Dog to the family is an application fee of $50, equipment fee of $250 and the cost of travel, accommodations and food for the 5 days of team training in Cambridge. All food and veterinary costs are born by the recipient once the dog has been placed with the family.

Must I fund-raise the full $30,000 to fund my dog before my child will receive his dog?
Fund-raising to support NSD is voluntary. Your support is greatly appreciated and instrumental to our success, but in no way effects if or when a service dog is received. The timing of a service dog placement is determined by chronological position on the waiting list and the availability of the right dog for the child's needs and situation.

Is NSD a registered non-profit charity that can provide donation receipts for income tax credits?
    Yes. Charitable Donation #: 898029228 RR0001
