Here are some common questions about the application process: National Service Dogs is currently accepting application requests from families living in Ontario and Alberta. This is a temporary measure and we will be re-evaluating our waiting list in the fall. If you have already received an application from us please forward it to our office. All outstanding applications will be open to review. If you are already on our waiting list or a graduate, please know our services have not changed regardless of where you reside in Canada. If you reside outside of Canada, please visit Assistance Dogs International at www.adionline.org for a listing of other service providers What is the process to apply for a Service Dog? First, potential clients submit a letter, by mail or on-line request form, outlining the effects of autism/PDD on their child's life and how they feel a dog would help the child in their daily life. Upon receipt of this letter, NSD will assess the information provided and determine if an application will be sent. Children must be between the ages of 2 and 8 and have a diagnosis of autism or other related PDD. If the family is eligible an application will be sent out by e-mail or regular mail. The application includes a number of components:
Once the completed application (including references) is received, the client will receive a letter acknowledging receipt of the application. The application will be reviewed and if successful the applicant will go on our interview list. Next an interview will be set up in the family home at which a trainer will bring a dog and meet your child, and discuss any further questions or concerns. Final acceptance is determined following this interview and families maybe disqualified at any time durng the application process. Once approved for a service dog families will have the opportunity to attend a general information session. This seminar will give families the opportunity to learn more about NSD and prepare them for the next stages in the process. Families who are not in driving distance of Cambridge will have the information mailed to them. How long does it take to receive the dog? How is NSD funded? How will our Service Dog be funded? The actual direct cost of the Service Dog to the family is an application fee of $50, equipment fee of $250 and the cost of travel, accommodations and food for the 5 days of team training in Cambridge. All food and veterinary costs are born by the recipient once the dog has been placed with the family. Must I fund-raise the full $30,000 to fund my dog before my child will receive his dog?
